Erosion of Acetabular Cartilage after Bipolar Endoprosthesis -Comparison Analysis of Femur Neck Fracture and AVN of the Femoral Head-

Abstract
Purpose: To evaluated the effectivess of bipolar endoprosthesis, we analyzed the erosin of acetabular cartilage after biplar endoprosthesis in patients with femoral neck fracture or avasculr necrosis(AVN) of the femoral head.

Materials & Methods: We retrospectively reviewed the medical records and radiographs of 47 patients(50 hips) who had undergone bipolar endoprosthesis for femoral neck fracture(25 hips) or AVN of the femoral head(25 hips) between January 1990 and July 1995. During the follow up periods ranging from 5 years to 9 years and 7 months, the results were clinically analyzed by the Harris hip score and inguinal pain. For statisticall comparison of acetabular cartilage erosion, we evaluated annual x-rays of acetabular cartilage taken from immediately after srugery until last follow up.

Results: The average Harris hip score was 83.2 points in femoral neck fracture and 87.3 points in AVN of the femoral head. Of the 6(24%) femoral neck fracture cases which presented more than 50% acetabular cartilage erosion, 1(4%) had occurred within 3 years of the operation and 5(20%) within 5 years. In AVN of the femoral head, among the 17 cases(68%) presenting more than 50% acetabular cartilage erosion, 7(28) had occurred within 1 year of the operation, 3 cases(12%) within 2 years, 4 cases(16%) within 3 years, and 3 cases(12%) within 5 years. Conclusion: The results verify/confirm(Ed-or, ¡¯We conclude from these results¡¯) that the erosion of acetabular cartilage after bipolar endoprosthesis in femoral neck fracture was superior to that of AVN of the femoral head, a finding which suggests that although bipolar endoprosthesis is not a successful treatment of AVN of the femoral head an indication of its use may be considered.
